Care management can range from a one-time fix of immediate needs to an ongoing relationship that advocates for and ensures that clients receive the care necessary to optimize quality of life, even in different residential settings – from living at home to skilled nursing settings. Care management maximizes independence and prolongs the ability to remain independent. Care management reduces stress for caregivers, both those living nearby as well as those living at a distance. An Age with Ease specialist will be with you every step of the way.
To begin, a complete and thoughtful assessment and evaluation is conducted by one of our specially trained staff to assess strengths and areas of need, including input from loved ones, family, and professionals. From this assessment, a customized care plan is created.
