A personalized assessment and evaluation is essential to ensure your or your loved one's lifestyle, preferences, and current situation are fully understood and considered in planning to support them to Age with Ease. Our evaluation designed to:
- Optimize ability to enjoy good health,
- Improve overall quality of life,
- Reduce or delay the need for hospitalization and/or institutionalization
- Support independent living for as long as possible.
You are not like anyone else. Neither are your needs. When you call, we will listen to your concerns and work with you, your loved ones, family, and medical providers to evaluate available options.
At your convenience, we will set a time for an Age with Ease specialist to come to your home to review aspects of your lifestyle and residence that impact your ability to live a safe, healthy, and independent life. Our specialized assessments are broad in scope and seek to capture a complete picture of your current situation including:
- Medical and emotional health
- Social connectivity and participation in activities
- Home safety and maintenance needs
- Independent living supports and gaps
- Financial resources and potential need for assistance with money management
- The status of your legal matters, including wishes for end of life care
- Concerns for the future that may be causing fears about growing older or declining independence.
Assessments help to bolster existing supports and identify areas for improvement. Age with Ease looks for solutions that honor your wishes and are least intrusive to your lifestyle. It is important to remember that while family concerns are often for the safety and protection of their loved one, the older person is may be focused on maintaining their independence. We view both concerns as equally important.
